Well, since you asked for well-designed, I think the rabbit style is awful. I usually use a double hinged Pulltap. The Laguiole is a classic version of this design that spans a large price range. The Code 38 is an ultra luxury engineered version of this. If you're opening older wines, an Ah So can be valuable. The Durand is a neat hybrid of corkscrew and Ah So that can be useful for very old friable corks. The Cork Pops is another very useful tool for dealing with old corks. Finally, there is always the shoe method.
This is basically what those CO2 bottle openers do, except they use food grade clean CO2 instead of some grimy bike pump, and they probably cost more than a good bike pump.

First off, how was it stored? Wines like to be stored on their side, in a dark place, with a low temperature fluctuation, and somewhat cool.
What state is the bottle in? Any leakages? How full is it?
Also, what is the winery? It'd have to be a pretty decent bottle of wine to begin with to last 28 years.
If you decide to open it and try it, look into a cork puller or a cork pusher, because a traditional corkscrew would probably destroy the cork, and you'd have to strain it out :)
I use these almost exclusively; I have one my great grandfather bought in germany after the war that I use every day.
This is what I use on questionable corks, IMO it can't be beat but I'm sure someone will tell me they're a bad idea! Anything more than 20 years old I use it.
