When we got the extra small size collar, we still had to pin it smaller. Honestly we ended up using a ferret collar while she grew into her collar, so you could try that, or maybe a kitten collar would be small enough. Weirdly the extra small harness we got fit her fine.
We used the Top Paw brand with the little reflective paw prints on it for her collar, leash, and harness (although the collar was too big, so we switched to the ferret one). I think they have them Petsmart (collar and leash), or here's the harness in pink on Amazon sold by Petco. Here's the green harness at Petsmart, but it looks a little different so I don't know if they've changed it or only changed the color of the buckles...
I like them pretty well because they're the thinnest straps we could find and didn't seem too heavy for such a small pup. For the most part they've lasted pretty well, although her brother ended up chewing through them all so we had to replace her collar with a leather one when she was big enough. We still use the harness and leash, though, I think we just bought replacements and kept them out of his reach.
My biggest tip is NOT to use retractable leashes. They're easy to drop, and then the dog thinks they're being chased by something that makes a scary noise...let's just say we learned the hard way, and have never used another one on any of our other dogs. It was pure luck that nothing bad happened to her, other than being traumatized.
