With 6 inches of girth a 64mm condom would be a good starting point. You might prefer the looser fit of a 69mm condom or the tighter fit of a 60mm. Condoms under 60mm are probably too tight, and there aren't any condoms larger than 69mm.

Pull foreskin down, put condom over it, done. Your problem is probably more with too small condoms being hard to roll down on your big dick.

It's going to be worse in North America than Europe, since North American manufacturers aren't allowed to produce larger sizes. Costs will vary depending on how many you buy at a time. For example, here's some for under $1 per condom but if you buy smaller packs it'll be more than that. Honestly the bigger concern is shipping time. It would probably be wise to have some on hand ahead of time, since if the opportunity arises she probably isn't going to want to wait 2-3 weeks.
