Maybe get him a decent pen that uses refills. I had a fancy Montblanc pen years ago, that thing lasted me probably 20 years until I finally ruined it. Bent over and it fell out of my pocket, then ran it over. Almost made me cry!
Montblanc pens didn't used to be SO expensive, and now I work from home, so seems silly to spend that money. So I bought a couple of awesome Pilot pens. They're about $13 and feel extraordinarily well made, and they write nicely too. Id suggest a Pilot Metropolitan, or if you need something a bit smaller, a Fisher Space Pen. Both are available in different colors/styles, including fountain in the case of the metropolitan.
I believe that they will fit in a pilot metropolitan gel body. There won't be an eraser though.

I don’t use the key ring knife as much anymore. I used to work in a place where I had to open a lot of boxes with customers around and sometimes people would freak out if they saw a knife. But the SOG micron was small enough that most people wouldn’t notice and if they did it wouldn’t freak them out. That’s why my regular knife is so small too (and because it’s super thin compared to most EDC knives so you don’t feel it at all in your pocket). I used to carry a Kershaw link a lot but that one was big enough that management didn’t like it when I pulled it out in front of customers.

The pen that I would recommend is Pilot Metropolitan Collection Gel Roller Pen. It has a nice looking partially metal body and takes G2 and Precise V5 RT refills quite handily. There are also a few other styles for the pen if you just search Amazon or jetpens.com. I just tested it out and it has the same unnoticeable small amount of tip wiggle as the standard body for the pen.

In terms of the thinnest of these... I think you'd like the Machine Era Co. and Pilot Cavaliers -- they are nice and slim, with a decent amount of heft -- both are well-made.

edit: oops... looks like this one isn't a fountain pen. anyway amazon is where i've gotten mine before. the "retro pop" collection goes on sale occasionally.
