A Razer DeathAdder.

It's made for gaming, but I really like the mouse. I've heard a few complaints about durability, but I've had mine for awhile and have not had any problems. I also got a really good deal, I don't know if I would want to pay the normal price.

I would highly recommend the Razer Deathadder.
Most Razer products are overpriced and not that great, but I really like the deathadder. Simple and reliable. I would also recommend using a mousemat like this with it.

I really don't know why you'd be getting pains so I can't help you there.

But I do want to point out that there are Deathadders without the rubber grip if you want to try those out.

Personally, I use the Steelseries Rival 300 and it's the most comfortable mouse I've owned. It's the same basic layout of the Deathadder, just a bit larger. It does have the rubber grips, I'm not sure if they feel the same as the Deathadder grips though.

I love my Razor Deathadder, but Razer quality control has a lot of issues, now more than ever. Any of their newer products, typically branded as "Chroma" are horrendous. They're cheaply built and seriously prone to failure. Get the DeathAdder Black or DeathAdder 2013. Those are much higher quality kit, though harder to find and as such, pricier.

As for Non-Razer kit, Logitech, Steelseries, and Zowie have the best rep for mice. Here's a few recommendations based on what I've heard here and read from reviews from Tom's Hardware, PCGamer, and Amazon. If you can, go up to Best Buy and or Fry's and get your hands on the mouse you're looking at. Everyone has a different preferance in Ergonomics. I cannot stand Logitech mice for their feel, but many love them and they're quality kit, as an example.

Headphones first:
Don't buy a headset. A "headset" is composed of a cheap pair of headphone drivers and a cheap microphone wrapped up in a chintzy, "gaming"-flavored package full of LEDs and shit, that gets sold at ridiculous markups. They're heavy and clunky, and honestly sound like crap. I find headset drivers to be almost too bass-heavy and their sound gets muddled. Listening to music on them sounds like you're listening to a recording from a nightclub, and when you play games with them, it's hard to separate one sound from another.

Instead, buy a nice pair of headphones, and a decent desktop microphone, and you'll not only spend less money, but you'll have better sound and a clearer recorded voice. I'd opt for a set of open-ear headphones, because they let you hear your voice as it normally sounds (put in a pair of noise-isolating earbuds or wear a pair of noise-isolating or noise-cancelling headphones and try speaking, you'll sound odd to yourself). For a ~30 Pound price range, I'd recommend these or these.

As for the microphone, just get a lapel microphone (like this one), or a desk microphone like this one (which is the Amazon best-seller for desktop microphones). Or you could get an around-the-ears microphone like this one, that you could then put your headphones over.

Next, for a mouse:
This depends a lot on personal preference. I'd recommend the Razer Deathadder, not only because they have one of the best sensors on the market (maintains perfect tracking even at extremely high speeds with no intrinsic negative acceleration, has adjustable DPI from 400 to 3500, etc.), but also because it is comfortable to hold and comfortable to use. The buttons are satisfying to click, the scroll wheel has just the right amount of tactile feedback, the cable is braided and flexible, etc. This mouse is 47.99 Pounds on Amazon (same link as earlier), but even with a budget three times as high, this is still the mouse I'd buy.

I find the R.A.T. mice to be gimmicks. Yes, you can twist some screws to change their length and whatnot, but the sensors are rather crappy ("DPI" means nothing to the quality of the sensor, you can get a $20 mouse with 3000 DPI; I only use 800 DPI anyways), and I'm also not a fan of how they look/feel. But, as with all mice, this comes down to individual preference.

Read the reviews; definitely valid reasons why it has a 3.5 star rating. IMO, it's akin to EVGA leaving off the thermal pads. It might not happen consistently, but once you see it, you see a clear manufacturing problem.

I use the Razer Carcharias Headset. It's got a decent microphone and the sound quality is great. Sounds good in games, can hear footsteps and explosions are bassy. Music sounds great too. They go around the ear and are open back, they are really light too.

It is suited for a palm grip. You might need to check which grip you are if you already own a mouse as different shape mice work better on specific grips. This mouse is great for FPS games but doesn't have many extra buttons. Depends what you are going to be playing.

Honestly, I couldn't recommend the [Deathadder] (http://www.amazon.co.uk/Razer-Deathadder-Ergonomic-Gaming-Mouse/dp/B00ABS62C6) more. It's one of the greatest mice I've ever had,and it fits my hand great. However, if that's not your thing, the Roccat Kone XTD optical is pretty great, and has tons of utility-options.
