My husband is Type 1 diabetic with moderate to severe gastroparesis (if he vomits once, he ends up hospitalized for at least 7 days trying to get it to stop).
I just wanted to chime in and mention two things that have been immensly helpful for him:

  1. Probiotics - like this one (make sure to refrigerate!). My husband uses the 100 billion culture.

  2. Bison! - bison is INSANELY low in fat and ridiculously delicious! My husband gets quite bored with chicken and soups, so finding bison was like winning a prize.

Here is the one I use. I had read that 30 million or more is best, and it has to be the 10 strain version or more. I was going to try 50 million if this didn't work, but 30 works for me. I store them in the fridge, though I don't think it's necessary.

It's really hard to recommend supplements to another person and say with certainty that they will help, especially because there are so many potential culprits. If you suspect you're dealing with a hormonal imbalance you should see a doctor, that's not something you want to tinker with on your own without consulting a professional. I don't know your gender, but a lot of women swear by Spironolactone for treating hormonal imbalance.

You're right, antibiotics often do little if anything for the actual cause. I can tell you from very recent experience that it took me months to realize that both topical and oral antibiotics had only exacerbated my fairly minor skin issues. After looking up decent probiotic supplements, I started taking this one 2x daily (I keep it refrigerated to preserve potency) and I also drink kefir. If you'd like to avoid dairy there are non-dairy options such as coconut kefir.

Another thing I did was drastically, and I mean DRASTICALLY, reduce the amount of sugar, particularly refined sugar, I was ingesting. The combination of these two changes, along with my now Clyndamicin-free Curology formula, has significantly helped my skin - though none of it happened overnight.

That's just my experience though, to be safe you should talk your doctor before making any major changes to your diet and/or taking supplements.
