It's so you can get the socket on the clamp nut when it's at a weird angle. You put it on the socket, and then plug the extensions into it.

Just use 2 8"inch extensions on each side, with the sparkplug socket obviously at the bottom. Took me maybe an hour, as it was the first time I had to do the job. I could probably get it done faster now.
