My little one is only one, and I started hrt 3 months into this pregnancy (due any day now). For now with the 1-yr-old have been sticking with daddy (lest he out me to family), and plan to let them choose what to call me when they get a bit older, unless it starts causing problems at some point...
As far as actually telling them, you could try a children's book like 10,000 Dresses to help the younger one. Simple is better. I once heard (probably here somewhere) a super-simple explanation that someone gave a child about a trans person. "They used to be a boy, and that made them sad. Now they take medicine to be a girl and that makes them happy." It doesn't get any simpler or truer than that, and how I plan on addressing questions at first when they are old enough to ask.

I strongly recommend the book Stuck in the Middle with You. It was a great read and provided some nice insight.

Jennifer Flynn Boylan has the book: "Stuck in the Middle With You" I didn't read this book because I fall into the 'our children are grown category'. However, I read her other book, "She's Not There" and thought it was fabulous. Maybe give it a shot! :)

There's a book called Transgender 101 that's very good about explaining most everything you'd need to know. There are also some books by Jennifer Finney Boylan that talk about her experience as a trans MTF parent that might be interesting. That's more of a story than an education resource though.
