Yes, It's not terribly hard and the instructable link below is about the best. The most difficult part is actually the counterweight which is not covered. You will need to have a rather large weight, which is dependent on the weight and lever of the top and also the placement from center of the counter weight. I suggest a cheap prototype, because it's difficult to get right on the first attempt.

I don't know of any plans but you could probably incorporate the mechanism used for lifting coffee table tops into a traditional desk design.
