Mcminn's is fantastic. Cadaver photos, really helpful. Libraries will have it as a reference text but some will lend it.

Actually there seems to be an updated version for it. Here. I found this to be fantastic for stepping through problems.
Its definitely aimed at a hospital based doctor, so you may not be able to do everything it says, but there should be enough information to settle your nerves.
