Looks like callamanus worms. You should assume all of your fish have been infected.
I fought these worms with my rainbow fish and I followed this website’s instructions
I got the levamisole from amazon and used a food weight to measure the correct dosage.
Just as a FYI, when I treated the tank, the fish got lethargic and my assassin snail stopped moving. Everyone returned to normal after I did the water change (per the instructions). Good Luck!
