It's an extender that attaches to your bra straps and wraps around to fasten at the front, hiding the back of the bra. I won't lie, it's not the most comfortable thing in the world, but it was easier than spending ages and £££ on a backless bra!!
The picture looks like it might just be using a backless bra convertor. If you search for that you should be able to find an online shop selling them in your country.
