I've had this most of my life. What worked for is allowing myself regular breaks away from screens, and just getting up and moving around. Is it a home office you're working at? If so, maybe try a standing desk. You also could try using glasses that filter blue light (Link). My brother uses them for similar symptoms and eye strain, and they work for him. I also would consider drinking more water, it's simple but often overlooked.

Because I got this question yesterday, I'll go ahead and address it. I moved from retail to work for a good friend of my family. He owns a local business that's been around since the late 1930s, and was started by his grandfather. I know it’s nothing high performance, fast, or crazy expensive, but the desktop and monitors came with the job, so I can’t complain too much. Please ignore the mess of wiring underneath. It was set up when they gave me my office, so I didn’t get to run things like I would have. We will be relocating soon, and with that, I’ll make things look much cleaner. Hopefully, I’ll be upgrading soon to something not running Windows 7 Professional, and something not this, because this can barely even run photoshop without crashing the graphics card. Oh well!
