I personally wash my cloth pads hot, and use mostly unbacked pads or pads backed only with fleece or microfleece. I wash them with my towels (and dish rags and other such items) in a heavy hot load. If they are PUL lined this may shorten their lifespan, although quality PUL should withstand normal hot washing for quite some time. If you use PUL, you might find that using a shield and insert type system (like Lunapads has) works for you because the PUL-containing shield becomes soiled less frequently, or you might decide to use something lined with microfleece or windpro fleece (or nothing, if your incontinence is light) as an alternative.
There's also cheap underwear [just lined with PUL]

If your pads contain synthetic toppers and absorbent materials like bamboo, microfiber, etc be aware that these are harder to thoroughly clean than something that is pure cotton or cotton and fleece only.
Okay, if we've addressed what you're using, we need to look at how you're cleaning them:
(http://www.fluffloveuniversity.com/how-to-wash-cloth-diapers/dealing-with-hard-water-and-cloth-diapers/) If you do, using calgon or borax in the wash is likely necessary for pads that have urine on them.
(http://www.fluffloveuniversity.com/how-to-wash-cloth-diapers/detergent-index/) A lot of brands unfortunately recommend washing routines that are far too mild. You want strong, standard detergent in an amount suitable for a heavy load.
(http://www.fluffloveuniversity.com/troubleshooting/solving-stinky-diaper-problems/how-to-strip-your-cloth-diapers/) your cloth pads. Once you do this and get into a good washing routine you should never need to do it again.
Let me know how it goes and if there's anything else you need to troubleshoot.
