You could tilt your head very far back and make sure the water doesn’t stream down your back. I have long hair so maybe that helps in directing the water away from my body, but many shampoos, conditioners, and body products can break my body out BADLY.

On that note, also experiment with different conditioners/shampoos that may actually be triggering it or making it a lot worse. Look up fungal acne ingredients and comedogenic ratings on ingredients in your shampoo/conditioner. Your body wash could be worsening it as well. I have had luck with [Peter Thomas Roth Glycolic Wash], although it does have glycolic acid so your skin may purge slightly at first for a bit.

Topical aloe vera can be helpful as it is anti-inflammatory, anti-fungal, and anti-bacterial, and also contains some natural salicylic acid that exfoliates inside the pore, even at a neutral ph.

Aromatica makes a good formulation that is 95% aloe vera, and does not appear to contain any fungal acne triggers or comedogenic ingredients. Some people are sensitive to aloe vera though, or other plant extracts so YMMV. Other formulations definitely have acne triggers in their formulas, so if you go this route be thorough in cross-referencing ingredients.

You can get chemical peels done on your back and shoulders, but those are better in winter when your skin won’t be exposed to the sun. I get TCA peels for $50 at a plastic surgeon’s medical spa side of the office. Salicylic acid peels could help as well.

As far as supplements: Lactoferrin specifically the Aptolactoferrin form because it binds to iron, robbing bacteria and yeast of it, accordingly inhibiting biofilm formation, growth, and also acts as an immunomodulator on human cells. Some people have luck with Zinc. There have also been studies that particular strains of probiotics that can have a positive effect on acne, and immune and inflammatory response.
Selenium, also N-Acetyl-Cysteine NAC assist in glutathione production and reducing inflammation systemically.

Responding with the cosmetic aspect of it first because if something topically is the root of what is triggering or worsening the acne, supplements can be a steep uphill battle over what possibly could be just a mild one, if any at all.
