Well - horrible as it is to contemplate it, you CAN actually fight and win a nuclear war with a nuclear-armed opponent so long as you're prepared to absorb some casualties. All-out global thermonuclear warfare doesn't kill everyone nor hit every place on earth.

On thermonuclear war by Kerman Kahn and The Evolution of Nuclear Strategy by Lawrence Freedman will take you through the mechanics. I thoroughly recommend them both - they make fantastic reading (although the second is the more readable of the two).
