I found these Bezi Bra Discs on Amazon and am a customer for life! I have large breasts (30H) with prominent nipples, and the "hide them in the seams" thing does not work at all for me. I can nip out through a Panache Porcelain bra (lightly padded). I wear Bezi bra disks. They don't add any bulk or pointyness, but they really keep nipples from showing. I have worn them under mesh bras (back in my 32FF days), and they are invisible under tight shirts.
Stick 'em in your bra and wear whatever.
