Folks. Get a pimple lancet. It’s about ten bucks, and saves me so much time and pain. Use the loop side 10 times more often than the pointy side. In fact, avoid the pointy side forever, if you can. Sterilize both sides with alcohol before and after every use.

2 experiences. 1st was during a facial, the esthetician made a small hole in the skin with a lancet and pressed it out. Excellent results, no scarring, no blood. Not everyone is qualified to do this, apparently. It's determined by state licensing laws.

2nd, NOT RECOMMENDED!, my dermatologist wouldn't remove them (wtf?) and said they'd eventually go away on their own, particularly if I rubbed them regularly (liar!). So after months of rubbing and no fewer milia, I did it myself with sharp tweezers to poke the hole and pressed it out with a comedone extractor. Ok results. No scarring but I did have a small bruise from pressing and poking too hard.
