Ubuntu is probably detecting issues reported by the SMART monitoring of the disk itself.
Get a new disk NOW.
You can use a SATA to USB adapter like this one.

And Clonezilla to clone your existing disk contents directly to the new disk.
Then just swap the new disk into place.

Basically

something like this will do, but your drive will be completely exposed and look kinda jank. If you're okay with that, this is a very cheap solution.

If you want it to be a bit more protected, look nicer, and pay a few more $$$ go with an enclosure like u/eron97 recommended.

edit: I think that linked enclosure is actually for 2.5" SSDs vs what you have, I think most HDDs are 3.5". Someone else here may be able to confirm

I was gonna suggest a simple adapter, but that enclosure is cheap enough that I'd just spend that extra few dollars for additional physical safety.

I’m not sure about batteries but here are the other components you need to upgrade the hardware. I used the software “SuperDuper” to transfer my HDD to my SSD.

Depends on if your laptop supports 2 drives or not. If not, I would recommend buying a cheap enclosure or just a USB adapter. Then you can use it as an external backup after you are done.
