I used this case, though it appears to be out of stock for Amazon Prime. I then used 3M strips to attach this USB 3.0 hub to the top of the power supply. The PSU had a couple of Molex plugs and I took the 12V and Ground wire and used some electrical twist caps to attach the 2 wires coming from the USB DC jack to the PSU. I then used this panel mount to go from the USB hub to the rear of the box then used that to connect to the computer.

To connect the 5.25" devices I used these and these. Though keep in mind, that particular USB to Sata adapter doesn't work for optical drives for some reason but it works for hot swap. Oh and I also replaced the 80mm fan in the case with a quieter one and a low noise adapter left over from a Noctua fan.

If you have any questions about how I did it feel free to ask! The only odd thing I have found is that if I try to power on the computer with the box turned on, sometimes I just get a black screen. If I turn the box off (it has a power switch on the front) and then turn the computer on it works fine. I wonder if it's trying to boot from one of the drives...
