HL2 Deluxe Collectors Edition at launch
I bought it for maybe $60 for the MIL to gift to me that Christmas, and the only awesome thing about it was the box.
The shirt was basic, with a little lambda on it. Pretty sure I wore it never.
The strategy guide was only a sampler of the first few chapters.
The game gave me headaches and nausea if I played for too long.
Once I got beyond the $40+ of disappointment, I enjoyed the heck out of the game, even if there was a little bit too much jetski driving.

Edit to add: pretty sure I have NIB physical copy of ep1/2 that I later bought through a steam sale of orange box IIRC.
Same thing happened to me with HL2 Collector's Edition. The manager didn't catch it until after I'd paid for the game and was almost out the door. He practically begged me not to tell anyone. Glorious! :)
