Some ideas here
If you google blended/pureed diet elderly you will find a lot of resources with good ideas.
I'm sorry you're having such a tough time! Learning about eating well can be really overwhelming. I've struggled with gaining weight at times too, so I can relate. I agree with some of the recommendations that have been posted so far (more frequent small meals, grains, bars, multivitamins).
My suggestions:
There are very high calorie drinks like this That are made specifically for weight gain. They aren’t going to make you gain 10 lbs over night but they’re 1/4 of the recommended daily calorie intake for the average person. If they don’t know they’re drinking them they could definitely lead to weight gain.
As stated already, more calories in than out. But to add to this, I found this to be very helpful. Boost VHC, it has high calories and if mixed into a shake or even just adding milk essentially masks the taste. One or two of these a day and you should see results in no time.
I don't know if they are actually Keto or just low carb (I don't follow keto diet, but like low sugar options), but the Purely Inspired protein shakes are vegan, 120 Cal, 20 prot, 1 net carb.

Edit: some of the reviews say they aren't vegan but I have them in hand and don't see any animal ingredients in the ingredient list.
Edit 2: some of the reviews mentioning dairy might be based on some of their powders or older formula. The RTD says dairy free and I read every ingredient to make sure I wasn't giving bad suggestions.
I don't think it's necessarily bad practice to have liquid calories/carbs, but that is a lot of carbs in one drink. I have been hunting for an easy on the go drink and am currently enjoying eas advantedge drinks when I need something quick and on the go. Only 4 carbs per serving, 2 g fiber, which amounts to 2 net carbs. They are pretty good, too.
If you want something convenient, I would highly suggest something like these over muscle milk. Way less carbs.
Try adding some extra protein and a healthy fat like avocado or walnuts
Purely Inspired Organic Protein RTD Chocolate, 11 Fl Oz (Pack of 4) https://www.amazon.com/dp/B07H9SRBXB/ref=cm_sw_r_cp_api_i_Lj0LDb34CEFJP
I like these shakes from Vega. under 200 cal and only 7g of sugar. https://www.amazon.com/Vega-Protein-Nutrition-Gluten-Chocolate/dp/B079HY3FDS
I'm not a huge fan of whey, so i stick to vegan protein sources (for supplementation, i still eat meat)
They aren't the cheapest either, but i buy them from Sam's club and they're a little cheaper. My mom actually gets Premier Protein shakes from there and they are cheaper, but whey.
According to the FODMAP app these are safe, I have yet to to try myself tho. Ordering some now actually lol
Orgain Grass Fed Protein Shake, Creamy Chocolate Fudge, Non-GMO, Gluten Free, 11 Ounce, 12 Count https://www.amazon.com/dp/B01CI57V2O/ref=cm_sw_r_cp_apa_i_ENdbBbJ1QSWGC
They're all pretty much the same. You can get them at any Asian grocery. Here is a amazon link. It's a lot cheaper if you get it from a store. It's essentially a natural MSG from mushrooms.
I keep a case of Premier Protein Clear Drink on hand for days when I don't get enough protein. It's 20g per serving, with just 2g net carbs. Only 90 calories because it's 0g fat, so it wouldn't sustain you if that's all you were drinking. They carry shakes too, which are higher calorie.

I drink these shakes called Evolve that are plant based protein, no dairy in them at all. They're really good! Here's a link to some on amazon, though I haven't tried this flavor: https://www.amazon.com/Evolve-Protein-Shake-Mellow-Mocha/dp/B01MT0PWGZ
So for both whey and casein, I prefer Optimum Nutrition's Gold Standard or Dymatize Elite / Iso 100. My favorite flavors are:
Whey:
Casein:
One thing to keep in mind with respect to whey and casein protein powder tubs is that although $45-65 seems like a lot of money, gram for gram it is still generally a way cheaper protein source than even chicken breast. It is just a larger cost up front.
With respect to pre-made protein shakes, I love Core Power and Muscle Milk:
Premade protein shakes are definitely more expensive gram for gram than chicken breast, etc. But you pay for the convenience.
I use Dymatize ISO100 Clear (https://www.amazon.com/gp/product/B01M13ZV8N/). It's pricey, but the protein to calorie ratio is really good and I prefer the texture of it to protein shakes after a workout.
Premier Protein 30g Protein Shake, Caramel, 11.5 fl oz Shake, (12 count) https://www.amazon.com/dp/B07MJL8TPK/ref=cm_sw_r_cp_apa_i_ndElDbC5AR232
I've never used any of this stuff but I see this suggested a lot.
There is also this, which my keto cousin uses, and a quick google netted me these pre made shakes.
Just throwing it out there as an option... Atkins Shakes plus a scoop of Whey Protein Powder (I get mine at Costco, nutrition slightly different than this one). I do a drink for breakfast, a drink for lunch, and it lets me have a hearty dinner.
270 Calories, 4 net carbs, 39 grams of protein, 10 grams of fat per drink. If I get hungry or need a snack I eat sunflower seeds (in the shell, so it takes a while to eat them) or cheese sticks.
I've done a bajillion keto meals for work, but it's difficult to stick to them as I have to spend hours on the weekends prepping. These take some adjusting to, but it couldn't be more convenient.
These have always been my go-to, even pre pregnancy: http://www.amazon.com/EAS-AdvantEDGE-Control-Chocolate-11-Fluid/dp/B00ID6BMRI/ref=zg_bs_5782436011_6
0g sugar and I find they're pretty easy to get down. I struggled with some first trimester weight loss and they kept me from getting too low.
I've heard good things about this one but haven't tried it! I was thinking of switching to it from ensure so if you try it let me know!

> 'foods' (meal replacements shakes/bars, protein powders that could hell fill out breakfasts, Ensure type shakes, etc) don't have a nutrition facts label.
This doesn't seem right at all. Protein powder has a Nutrition Label, the same as regular food. So do protein bars and Ensure. Supplement labels are on vitamins and supplements.
Premier Protein is less expensive than Genepro. You can mix unflavored protein into anything!
Premier Protein Clear Protein... https://www.amazon.com/dp/B07B9DYMTX?ref=ppx_pop_mob_ap_share
530 calories a pack. You get 27 of them.
Idk if I'm allowed to post links but this is what I've been ordering:

EDIT: I think there are other sellers on Amazon that may sell her for a few bucks cheaper. Take a look around.
Actual dietitian here. This is what you want.
https://www.amazon.com/Boost%C2%AE-Very-High-Calorie-Vanilla/dp/B0089M3FLC
You have to consume ~500 EXTRA calories per day to put on a pound per week, btw.
I was mistaken with the price. $40 is for the 27 pack of 360 calories and $50 for the 27 pack of 530 calories. Here's a link on amazon: https://www.amazon.com/Boost%C2%AE-Very-High-Calorie-Vanilla/dp/B0089M3FLC
ERCP and gallbladder removal definitely helped! I had acute, not chronic, pancreatitis. It was called by a blocked pancreas duct, which is why they did the ERCP to put in the stent -- and then later to take it out. Definitely have felt better since!
Sometimes liquids can be easier than food. There are also 'clear' versions of those liquid protein drinks that are gentler on the stomach, at least for me. I don't really like the taste of regular Ensure much, but the Ensure Clear juices were okay.
There's also options like this: Very High Calorie meal replacement drinks. They'll give you a very high protein to volume ratio. Thing is, I don't know how gentle/rough they are on the stomach, and I would definitely want to track down just one to try initially. It'd suck to buy a whole pack of them and hate them. I'm just not sure where you'd find them locally.

These are a godsend, by far the best meal replacement shake I've tried. They're my go-to during stim runs now
I agree with the other commenters here, however I would not blanket tell you not to take doctor prescribed medications for anxiety. I would say to get at least two more diagnosis and recommendations before you start playing with chemistry. At the same time do as the other users recommended. Make the following commitments to yourself:
You'd be surprised what these changes can do for your overall emotional health.
Good luck.

I drink two of these every morning for breakfast (whether on the diet or not.) No problem losing 40 lbs. with those as my breakfast. May not work for everyone but they work for me. No milk, no sugar. (I get chocolate and strawberry for variety.)
